Our support workers provide specialist support for clients and families with more complex needs. We aim to break down barriers and reduce the level of complexity around accessing these services, while also helping families and carers build resilience and connect with other support networks.

A Person-Centred Approach

Here at Kelch Care, it’s all about you! We put our clients, families and carers first to ensure you get the most out of your NDIS plan. We aim to develop long-term strategies for NDIS participants and families to manage their own plans—on top of providing access to support services. Our support workers help by:

How it Works

We follow several steps to help clients work towards their individual goals and learning outcomes.

Step 1: Getting to Know Our Clients

Our support workers will sit down with you, your carer and/or family to identify and explore your needs and goals. From there, we can get started on developing your NDIS plan.

Step 2: NDIS Support Coordination

Step two involves working together to write a customised NDIS plan that outlines both your short and long-term goals as well as how you aim to achieve them. By putting pen to paper, it motivates you to work towards your goals and learning outcomes.

Step 3. Accessing Support Services

We’ll use our knowledge of the supports in your area to identify which support services will be most suited for your plan and how you can best access them.

Step 4. Getting The Most Out of your Plan

Once we’ve established and implemented your NDIS plan, our support works will regularly check in to ensure the plan is still meeting your requirements and providing you with opportunities to develop and achieve goals.

Step 5. Enjoy a More Confident You

Through accessing supports such as accommodation and tenancy assistance or transport and travel, you’ll develop the skills to become more independent and gain the confidence to manage your own plan.
